Restrictive Legend Removal Instructions
If you have held your Conquest Petroleum shares for more than a year, we can now remove the restrictive legends from the stock certificates. If you wish to have the restrictive legend(s) removed from your stock certificate(s), please adhere to the following instructions:
Step 1: Download and complete the Shareholder’s Representations Letter form. (To download this form, please
click here.)
Step 2: Draft an instruction letter for the Company’s transfer agent. Please include one of the following instructions:
(1) I, _(name of shareholder(s))_, request the removal of the restrictive legend from my stock certificate(s), dated _____ and numbered ______, and the issuance of a new stock certificate.
OR;
(2) I, _(name of shareholder(s))_, request the removal of the restrictive legend from my stock certificate(s), dated _____ and numbered _______. Additionally, I request the division of my shares. Please divide my shares as follows: Then, state the number of shares you wish to have allocated to each new certificate. (Please note: There is an additional fee for each new certificate.)
Step 3: Send via registered, insured mail (1) the completed Shareholder’s Representation Letter form, (2) the original stock certificate(s) representing the shares of common or preferred stock, (3) your instruction letter and (4) the appropriate fees to the Company’s transfer agent at the following address:

The restrictive legend removal fees are as follows:
Restriction Removal $75.00
Each New Certificate $30.00
Fed-Ex Services $25.00 (unless Fed-Ex pre-printed label provided)
It is recommend that you use registered, insured mail or equivalent courier service when sending or receiving stock certificates or related documentation.
